  • Open a general inquiry service request at bottom of window
  • Bullet No Specific participant or participant not in system and click Next
  • Under Select an Area for your inquiry select Other
  • Under Indicate what your request is about select Other
  • Enter in, “408(b)(2) plan fee disclosure request”

  • Call 1-800-835-5037, and reference the plan number and name as a plan administrator
